Fees & Payment Options

  • Fee Structure: Fees are based on the specific service provided and align with standard local rates.

  • Payment Methods: Cash, Check, or Venmo (@Joseph-Szlamnik; please enable privacy settings).

Insurance Coverage

  • In-Network: Optum Behavioral Health, Lyra Health, and Medicare.

  • Out-of-Network: Full payment is required at the time of service for all other providers. A Superbill / Statement of Services can be provided for HSA or insurance reimbursement claims.

Important Insurance Considerations:

  • Co-pays are due prior to or on the day of service.

  • Insurance coverage requires a formal mental health diagnosis, which becomes part of your permanent medical record.

  • Clients are responsible for verifying out-of-network benefits with their insurer beforehand.

Special Programs

Gambling Treatment: As an authorized State of California and UCLA Gambling Studies Program provider, treatment for gamblers and affected family members begins with an initial block of 8 free sessions.

Good Faith Estimate (No Surprises Act)

Cancellation Policy

  • 24-Hour Notice Required: Cancellations made with less than 24 hours' notice are subject to the full session fee.

  • Note: Insurance carriers do not reimburse for missed or late-canceled sessions.

Confidentiality Exceptions

All communications are strictly confidential under California law, with exceptions for:

  1. Impending danger to self or others

  2. Suspected child, elder, or dependent adult abuse/neglect

  3. Issuance of a valid Court Order

  4. Necessary collection actions for unpaid balances

  5. Required disclosures for insurance reimbursement
